Medical Oncology Code Changes

Anticoagulant Management
Effective January 1, 2007 there are two (2) new codes for anticoagulant
management services:
99363
Anticoagulant management for an outpatient taking warfarin, physician review
and interpretation of International Normalized Radio (INR) testing, patient
instructions, dosage adjustment (as needed), and ordering of additional
tests; initial 90 days of therapy (must include a minimum of 8 INR
measurements)
99364
Each subsequent 90 days of therapy (must include a minimum of 3 INR
measurements)
These codes may be reported for warfarin
management in the outpatient, office or freestanding center site of service.
Both the physician’s non-face-to-face services and face-to-face patient
encounters are included in these codes, such as:
o
Reviewing, interpreting and
ordering initial and repeat prothrombin time tests
o
Adjusting medication dosage
o
Altering and updating the care
plan
o
Documenting results
o
Communicating results and
instructions to the patient

The American College of
Surgeons Commission on Cancer (CoC) and the American Joint Committee on
Cancer (AJCC) have made available to all cancer care professionals the
Inquiry and Response System (I&R). This is a repository of thousands of
questions and answers related to registry data and cancer program standards,
which many are used in the development, clarification, and revision of the
Cancer Programs Standards or updated policies and procedures, expanded
documentation, clearer communications, and increased education and training
resources. The I&R team is comprised of CoC technical staff and physician
experts in specialty fields. You are encouraged to review the database and
submit questions by going to
